616 Mat Pilates Instructor Training Enrollment Agreement & Liability Waiver

By enrolling in the 616 Mat Pilates Instructor Training ("Program"), I acknowledge and agree to the following:

1. Enrollment

My enrollment is not confirmed until accepted by VL Collective LLC d/b/a 616 Pilates ("616 Pilates") and any required payment has been received.

2. Tuition

All tuition and fees are non-refundable and non-transferable. Enrollment is reserved solely for the registered participant and may not be assigned, transferred, or substituted. If I withdraw, fail to attend, or do not complete the Program for any reason, I understand that I will not receive a refund, credit, or transfer, except where required by applicable law.

3. Attendance & Certification

I understand that I must attend all scheduled training sessions. If I am unable to attend a session, I must complete any required make-up session(s) approved by 616 Pilates.

To receive my 616 Mat Pilates Instructor Certification, I must successfully complete all program requirements, including attendance, participation, practical evaluations, required assignments, and payment of all tuition and fees in full. Certification is awarded at the sole discretion of 616 Pilates based on successful completion of these requirements.

Completion of the Program does not guarantee employment, contractor status, or teaching opportunities with 616 Pilates. Graduates may have the opportunity to audition to teach at future 616 Pilates events.

4. Code of Conduct

I agree to conduct myself in a professional, respectful, and supportive manner toward instructors, staff, fellow participants, and the training venue. 616 Pilates reserves the right to remove any participant whose conduct is disruptive, unsafe, discriminatory, harassing, or otherwise inappropriate. Removal from the Program does not entitle me to a refund or transfer.

5. Assumption of Risk & Release of Liability

I understand that Pilates and fitness training involve physical activity and carry inherent risks, including but not limited to muscle strains, sprains, falls, illness, property damage, permanent injury, or death.

I certify that I am physically able to participate and will notify instructors of any injury, medical condition, pregnancy, or other circumstance that could affect my participation.

I voluntarily assume all risks associated with my participation and, on behalf of myself, my heirs, executors, administrators, and assigns, fully release, waive, discharge, and hold harmless VL Collective LLC, d/b/a 616 Pilates, its owners, officers, employees, instructors, contractors, volunteers, affiliates, sponsors, venue partners, and agents from any and all claims, demands, causes of action, damages, losses, liabilities, costs, or expenses arising out of or relating to my participation in the Program, including claims arising from ordinary negligence, to the fullest extent permitted by Florida law. This release does not apply to gross negligence, reckless conduct, or intentional misconduct where such releases are prohibited by law.

6. Photography & Media Release

I grant 616 Pilates permission to photograph, film, and record me during the Program and to use my name, image, voice, and likeness for marketing, advertising, educational, promotional, and social media purposes without additional compensation.

7. Intellectual Property

All manuals, educational materials, programming, videos, class sequences, branding, trademarks, logos, and the 616 teaching methodology are the exclusive intellectual property of VL Collective LLC.

I agree not to copy, reproduce, distribute, publish, record, sell, modify, or use these materials without prior written permission.

8. Program Changes

616 Pilates reserves the right to modify instructors, curriculum, schedules, or training locations as reasonably necessary.

If the Program is canceled or rescheduled by 616 Pilates, I will be offered enrollment in the rescheduled training dates or the next available training session at no additional cost.

9. Governing Law

This Agreement shall be governed by and interpreted under the laws of the State of Florida. Any legal action arising from this Agreement shall be brought exclusively in the state or federal courts located in Alachua County, Florida.

10. Electronic Signature

By electronically signing below, I acknowledge that I have carefully read this Agreement, fully understand its contents, understand that I am waiving certain legal rights, and voluntarily agree to be legally bound by its terms. I understand that my electronic signature has the same legal effect as my handwritten signature.
